The Federal Constitution Art. 117 para. 2 shall read:
1) The Confederation establishes the digital, redundant “Single Health Insurance” (SHI). It ensures that operations begin three years after adoption. Development of MEDIFACT MF, the electronic patient dossier EPD, and the necessary trackers begins immediately.
1a) Principle
The SHI is mandatory for all persons residing in Switzerland. Emigrants lose coverage; access to the EPD remains.
1b) Exceptions
Anyone who wishes broader coverage and can prove an ICA health insurance, which passes through MF and the trackers, is exempt from the SHI for its duration. Prices remain free. Alongside the SHI, an ICA policy for alternative medicine may exist.
2) Organisation of the SHI
The SHI consists of:
– MEDIFACT MF
– Electronic Patient Dossier EPD
– Financial Administration FA
The AHV number serves as insurance number and access code. Access is electronic or by telephone.
3) Personnel
Patient-facing personnel and laboratories declare their training and experience to MF. MF determines the fair compensation value.
4) Services
The SHI covers all KVG services and non-occupational accidents (NOA). Plus:
– no costs for minors
– no co-payments
– free choice of doctor except in hospitals
– breast cancer screening
– fair compensation of prescribed imported medication
– emergency and medical transport in Switzerland
– dental services excluding cleaning and unnecessary prosthetics
Privately paid single-room surcharges are permitted.
5) MEDIFACT MF
MF is the digital intelligence and control center of the SHI, making decisions neutral, fact-based and legally consistent.
MF assigns treatment numbers, verifies legality, directs treatments and operations and prevents abuse.
MF sets fair compensation for personnel, equipment, facilities and special skills, and arranges staff placement as well as cost-efficient family care.
MF provides service providers with the necessary trackers; these remain property of the SHI. They record all relevant services and patient contacts, forming the basis for automated documentation.
MF informs patients clearly about upcoming fair compensation. With their consent, payments are triggered. Data flows into the EPD.
6) Base rates per full hour
– Fr. 200.– for medical services
– Fr. 130.– for nursing services
These rates serve as base values. MF determines the effective fair compensation. Other KVG services are reimbursed at proven cost plus 20 %.
7) Monthly premium
Permanently Fr. 150.–. Inflation is calculated separately and balanced annually among the parties.
8) fair compensation system
MF calculates fair compensation based on tracker data, costs and flat rates. Co-payments and direct payments are prohibited.
9) Electronic Patient Dossier EPD
All health data as well as existing patient records must be integrated into the EPD. It must be consulted before every treatment. Access and changes are logged.
10) Efficiency
MF periodically reviews the efficiency of service providers and ensures sustainable financing.
11) Financing
Any losses are borne by the Confederation. The cantons no longer finance hospitals.
12) Financial administration
This collects the premiums. Automation enables a small workforce.
13) Further legal amendments
– KVG Art. 43, 49 and 49a are repealed.
– UVG Art. 7 para. 2 is repealed; it is replaced by OR 324a.
– Existing KVG insurers are dissolved. Reserves and patient data are transferred to the SHI and into the EPD.
14) Other laws, legal recourse
Unless otherwise specified, other laws apply. Legal recourse at the claimant’s place of residence remains guaranteed.
15) Transitional provisions
These are regulated by the Federal Council.
